At Lifetime Home Healthcare, we are a licensed 245D provider serving individuals across the entire state of Minnesota. We work in close partnership with Case Managers, MSHO care coordinators, CADI and Elderly Waiver teams, case management agencies, and hospital social workers to deliver high-quality, person-centered home and community-based services.

Our comprehensive service model includes ICLS (Independent Community Living Skills), Individualized Home Supports (IHS), Respite Care, Night Supervision, Homemaking, Adult Companion services, and 24-Hour Emergency Assistance. We are committed to clear communication, timely intake, consistent service delivery, and seamless coordination with county teams, families, and care professionals. Our goal is to promote independence, safety, dignity, and meaningful community participation for every individual we serve. We welcome professional referrals and prioritize collaborative, reliable, and responsive care for all clients.
